What is a Case Manager and what do they do?
The role of a Case Manager is pivotal within the social sector, especially in areas such as health, community services, and housing. They play a crucial role in improving clients' lives by ensuring they receive the right support at the right time. They help clients navigate complex systems, access essential services, and achieve greater stability and independence. Their work not only benefits individual clients but also strengthens communities by addressing social determinants of health and wellbeing.
On a day to day basis they will cover many aspects of their clients needs including:
Assessment and Planning – they assess a clients needs and goals and then create an individual plan that will outline the required services and support
Coordinate Services – they will connect their clients with necessary services such as healthcare, housing, education and social support.
Advocacy – they will advocate for their clients to ensure their rights and interests are being represented and protected.
Monitor and Evaluation – they will monitor the care plans and make adjustments where and when needed that will ultimately improve outcomes
Support and Counselling – they will play an emotional support role not only to their immediate clients but also to their families and help with coping strategies and life skills
To be a Case Manager you will need to develop a particular set of skills. You must be a very strong communicator and possess fantastic verbal and written skills. These are crucial interacting with clients, families and service providers. Additionally, you must have the ability to empathise as the role will be nigh on impossible without having the ability to build trust and rapport with clients and their families. You must also be very good at not only identifying problems but developing creative solutions that will improve your clients outcomes. Finally, but very importantly you will need to be super organised, you have to have very effective time management skills, have the ability to juggle multiple clients and tasks without letting service standard decline.

To be a Case Manager a typical background would be a background / education in social work, nursing, psychology or similar related field. Either way, you will usually have a background in helping people overcome obstacles in their life and improve life outcomes for those that are disadvantaged.
